The Town of Brookline Council on Aging is seeking an experienced and compassionate Clinical Social Worker II to provide administrative, supervisory, and clinical casework services to Brookline residents age 60 and older.

About Us

The Council on Aging works with town agencies and community partners to enhance quality of life for older adults through health, learning, arts, nutrition, socialization, and recreation programs at the Brookline Senior Center and throughout the community. We have a dedicated senior center facility which provides a rich, diverse environment with a wide range of opportunities, programs and services in the areas of health, learning, art, socialization, nutrition, and recreation.

Responsibilities
The Clinical Social Worker II will provide outreach and home-based clinical and case management services, conduct psychosocial assessments, and develop service plans to address client goals and needs. The position administers the innovative Homecare Program and makes referrals for medical, social, housing, legal, and financial services. The Social Worker provides crisis intervention, including suicide risk assessments and safety planning, and investigates referrals regarding elders at risk, abuse, or self-neglect in coordination with state and local agencies.

This role serves as an advocate and liaison with health care providers, social service agencies, housing providers, and other community organizations. The Clinical Social Worker II participates in interagency meetings, represents the Council at community events, assists with program development, and contributes to resource materials. The position also provides day-to-day supervision to volunteers, maintains confidential client records, and prepares reports.

This position is based at the Brookline Senior Center but includes frequent home visits to older adult residents, requiring local travel and the flexibility to work outside of regularly scheduled hours as situations may demand; performs all other related work as required.

Qualifications:

Master’s in Social Work required with expertise in Gerontology and two years related experience; L.I.C.S.W. required.

The ideal candidate will have a deep understanding of the services and resources available to older adults, along with strong knowledge of state and federal regulations that impact this population. They will be skilled in counseling, casework, and crisis intervention, and able to support individuals and families with compassion, empathy, and patience. Success in this role requires strong communication, organizational, and time management abilities, as well as the capacity to coordinate care effectively while maintaining professional boundaries and confidentiality. Proficiency with database and word processing applications is also important for managing records and reports.
